[az] Provider Refused Third Party Funds

Personal injury auto accident from over five years ago. In 2021 the third party claim was settled, and the provider (chiro) had an exorbitant bill for 15k. Services actual value approximately 3k. Providers had to reduce, and they were offered 5k which chiro refused opting to wait for UIM funds which they had prior knowledge of because they asked to see the declarations page during the consultation.
Fast forward to current day it's finally coming to a close. Provider doesn't have a lien, and so they are being given the same offer which they are refusing and playing victim and stating since the lawyers don't reduce they don't either. The bill is from 2019-2020.
What are the provider options? Injured no longer in AZ. The lawyer doesn't really care and just wants their money and says it's not their job at this point. They did send the offer but want their hands clean of it even though it doesn't seem they negotiated it properly in the first place during the third party portion. They basically said let him try which isn't very helpful at all and also something they would never do themselves. What should the injured do? The lawyers are waiting on their signature.
