[us, Wi] New Employer Asking For Medical History.

My new part time low paying job is asking for a full medical history and includes a clause basically saying I give them permission to contact past and current medical providers, insurance providers, and employers to also obtain my medical history. The company is not providing insurance at this time. It is a national fast food restaurant.

I have never been asked to give this information to an employer. I've even worked in an environment requiring yearly physicals because of hazardous exposure, but that info was collected by Healthcare providers and insurance and only reported need-to-know to the employer. Also having worked in insurance this seems like a clear HIPPA violation. The company will not let me complete my onboarding information without filling this out completely (online) and will prevent me from being paid.

Asking for advice on this. If the employer insists I will refuse. Why would an employer ask this? Do they have any right to other than if someone willingly consents? If I am refused employment do I have any legal recourse? If not, and applicable, how should I report this? TIA.
