Accused Of Falsifying Doctors Note

To put this simply, I was recently accused by the code of honor from my university of falsifying a doctor’s note because the student disability office claims they could not verify it. - I was very sick and seen by the ER the night before my absence, the original note I was discharged with said I was seen on the 2nd and could return to school on the 5th. - The next day I had to miss my class the (3rd) due to this contagious illness. I woke up to an email from my school with the absence verification form that included very specific wording that the doctor’s note should have on it. - So I called the ER and asked if I could have my doctors note updated with the correct wording/format the university wanted on it. The provider agreed, updated, reissued, and signed/dated it on the 3rd. The note still stated I was seen by the ER on the 2nd and that I could return on the 5th, but this time had the specific wording the university wanted on it. Nothing on the note was written or changed by me, and nothing on it was factually incorrect.

I am now having to wait for a committee meeting to say my side of the story (where they decide my fate). I am genuinely lost and don’t know what I could have done different in this situation. I’m not sure if they’re going to try to spin it like I did something wrong. I was depending on the providers expertise when it came to updating the note. This has been sort of eating away at me while I wait for this meeting. I would appreciate any kind of advice or what to typically expect from this code of conduct committee meeting.

Edit: I also have a written statement from the provider verifying the note. He has also personally reached out to the university to verify it.
