Addressing Substance Abuse In A Teenager With An Organ Transplant

My son who is now legally an adult is a transplant recipient (received a liver in 2017) and has been heavily using marijuana and not watching his health as needed. About a week ago, he drank an excessive amount of alcohol (the first occasion he has drank that I’m aware of) which is medically noncompliant with receiving a liver. For the above reasons I am concerned to the point where we are considering inpatient treatment, but are also hesitant due to concerns with the transplant team denying care if they know about the substance abuse.
Does anyone have any insights or experience on this topic? Any guidance is greatly appreciated.
