Dog Bite

I was bit by a dog 06/2022. My neighbors German Shepard escaped their yard and came up to me while I was mowing the grass and chased me into my garage/attacked me. There ended up being a few puncture wounds and bruising as the dog's jaw clamped down around my thigh. I ended up going to urgent care and then the er a couple of days later as it wouldn't stop bleeding and they gave me a stitch to help.
My neighbor (the wife) immediately came over and was worried. After I went to the urgent care she begged me not to tell her husband as he would put the dog down and she offered to cover medical. I reported it as a dog bite and gave their info to the doctor but never filed anything officially. The wife turned standoffish but did pay the few hundred in bills after my health insurance covered some.
I am in Ohio and have heard that you have 6yrs to file a claim against a homeowners insurance for a dog bite. I still have hard lumps under my skin from the bite and it took about a year or so for the bruising to go away. I am now absolutely traumatized by larger dogs when I never was before. I am looking to possibly get some therapy done for this. I don't know if I have a case to file though since her and I did discuss her paying the bills and she did.
