Got A Notice To Title Vehicle

Important information: my family moved to Florida 2 years ago after living in Missouri for 20 years. I have been attending college in Missouri since before they moved, and have not transferred my drivers license to Florida yet.

So recently (about 3 months ago) my cars engine started having trouble and I ended up selling it. The car was in both my dad’s name and my name (dad was a co-owner since he paid insurance on it while I’m in college). I went and bought a new-to-me vehicle and my dad registered it in his name down in Florida. About a week ago I got a notice to transfer vehicle title from the state of Missouri stating that the vehicle was not registered. I called the DOR (number on the postage I received from them regarding titling) and I was told to get the form 768 from their website which is a general affidavit and get it notarized at the dmv. My question is, what exactly do I put down on the form? It has multiple choices on there, and non of them match the situation I am in. There is a “other” option to explain, but what exactly do I write in that section?
