I Reported My Co-worker To My Hwd For Neglect.

I told her about some instances of a co worker who I witnessed neglect the cares of residents such as changing residents out of dirty or soiled cloths, not cleaning or emptying a catheter, not assisting residents to bed or to the bathroom, being cold and callous with residents, ignoring motion sensor alarms for high fall risk residents etc. I also told her what another co worker told me whitch solidified my beliefs that she was in fact disregarding the care of residents. The problem is, me and the neglectful co worker have had conflict in the past. To be fair, she has had conflict with many of the staff. On the other hand there are a couple of staff who she has befriended and may speak against me. I am worried that I may get accused of falsely reporting because of the issue and face retaliation. Should I go forward in writing a statement to my HWD or should I contact an ombudsman or revoke my report?
